Incident Narrative

An employee was preparing the roof for solar panel installation. The employee was transitioning from an extension ladder to the roof when he slipped off the ladder and fell approximately 12 ft. to the ground, resulting in hospitalization for four broken ribs and a concussion. The conditions at the time included snow and ice.
