Incident Narrative

An employee was cutting tilt rods for shutters using an electric table saw. The employee's left hand fingers contacted the blade, causing an amputation to the middle finger up to the nail bed with bone loss, soft tissue removal to the inner part of the index finger and a nail bed avulsion to the thumb.
