Incident Narrative
An employee was putting in pipes to install the supports for the concrete base. The employee ascended the rebar column approximately 18 feet above the concrete base. The employee was standing on a 2x4 piece of lumber that was inserted into the rebar when the lumber split and the employee fell approximately 18 feet. The employee landed on his feet on a 15x15 concrete footer and was hospitalized for fractures to both ankles.
