Incident Narrative
On January 15, 2021, an employee was operating a press when it was having difficulty coming down. A maintenance employee was dispatched and came to look at the press. The maintenance employee was in the back of the press and cycled the press as the other employee was in front of the press with her hands on the part. As the press cycled, it partially amputated her right thumb and caused crushing injuries to the hand.
