Incident Narrative

An employee was going to unload a tractor trailer at the loading dock. The employee straddled the trailer lip and the dock to release the door latch. The tractor trailer pulled away from the dock and the employee fell to the ground between the dock and the trailer. As the tractor trailer backed up to the dock again, the employee was pinned between the trailer and the dock.
