Incident Narrative
An employee was on a ladder re-installing a shaft assembly and bolting it to a polisher machine. As the employee was retrieving the bolts from his pocket, the rigging point on the shaft failed and struck the ladder causing it to tip over. The employee fell backwards off the ladder onto the ground and the shaft assembly struck his right lower leg. The employee suffered an impact injury to their lower right leg and a back injury from the fall to the floor.
