Incident Narrative

An employee unloading a crate attempted to turn the crate near the rear of a trailer so that he could get a pallet jack under the crate. A board broke off the crate and the employee fell out the rear of the trailer, landing on the ground and suffering a fractured pelvis that required hospitalization and surgery.
