Incident Narrative

An employee was rounding out the manway of a tank using a portable 10-ton air jack. The opening of the manway was 24" in diameter. The employee and a co-worker were using the jack along with wood shims to prevent the tank from being damaged. One of the wood shims shot out under uneven pressure and struck the employee in head. The employee was rendered unconscious and was hospitalized for a head injury.
