Incident Narrative

The injured employee was on the ground holding the bottom part of a sprayer while a coworker sprayed releasing agent onto concrete forms. The coworker noticed that the form was leaning toward the injured employee and shouted to warn him. The injured employee tried to run, but the form fell forward on his lower torso. The employee sustained a fractured hip.
