Incident Narrative

An employee was conducting preventive maintenance on a locked-out fryer exhaust on a roof. Airflow started to move the fan blades, which caused the belt to move. The employee's glove and index finger were caught by the belt and pulled into the pulley, causing an amputation to the finger.
