Incident Narrative
An employee was in a trench encased in concrete that was 4 feet in depth and 5 feet wide cutting an old pipe. When he cut the pipe, a part in the trench collapsed and struck the employee's ankle causing the employee to lose his balance. The saw then contacted the top of his boot and lacerated the top of his foot.
