Incident Narrative

An employee was removing the last chain from a substitute vehicle for a driver that had broken down. The driver went into the cab of the semi-truck and began driving forward. The truck ran over the employee's feet, and the employee was pulled against the tires. The employee was hospitalized with fractured ribs and crushing injuries to both feet and ankles.
