Incident Narrative
An employee was operating a welding machine when it was jammed by a piece of material. The employee cleared the jam and started the machine. The material jammed the machine again. The employee walked behind the machine and proceeded to clear the jam. Soon after, the machine continued to cycle. A set of material clamps came down and a material pusher came forward. The employee was then caught in the machinery and both of his hands were amputated.
