Incident Narrative

Two employees were moving material that was loaded on a material cart that weighed 3,500-4,000 lbs. The material cart was on an electric pallet jack. The injured employee tried to help stop the pallet jack and was caught between the material cart and a support beam, sustaining a ruptured spleen.
