Incident Narrative

An employee was moving a 1,000 lbs. 10' x 15' rubber mat using an all-terrain powered industrial truck (PIT). While unrolling and setting the mat into place, the employee's right hand was caught between the mat and the end of the PIT's fork resulting in amputation of the right ring finger.
