Incident Narrative

An employee was attempting to line up machined holes in a repaired gear box assembly's base plate to bolt it into the gear box cabinet. The gear box assembly was being raised into position and the employee was correcting the alignment when the gear box assembly shifted and caught his right hand between the upper interior cabinet and the gear box assembly resulting in amputation of the ring and middle fingertips.
