Incident Narrative
An employee was operating a tube bender and noticed that one of the pressure dies was misaligned. The employee initiated a manual pause on the bender and then started adjusting the pressure die. A separate pressure die actuated, catching the employee's finger between the pressure die and a tube die causing injury to the distal phalanx of the right middle finger which required medical amputation.
