Incident Narrative
An employee was inventorying drums of chemicals and assisting a forklift driver when the forks became stuck in an elevated position. While dislodging the mast chain, the employee's right hand became wrapped around the chain and was pulled into the pulley system as the chain realigned. The employee sustained lacerations, three broken fingers, and a partial amputation of the index finger.
