Incident Narrative

An employee climbed up an extension ladder and began using bungee cords to secure a tarp to a railcar. He hooked one end of a cord and then pulled the cord so it would reach the anchor point for the other end. The cord broke causing him to fall backward off the ladder approximately 10 to 12 feet to the ground. The employee sustained fractures to the right knee and left ankle, and his left heel was shattered.
