Incident Narrative

An employee was backing a stand-up fork truck out of trailer into the warehouse and struck the bumper of the loading dock at the outside edge of the dock plate. He fell off the fork truck onto the concrete floor in the dock area. The employee was hospitalized for a leg injury that required surgery.
