On October 21, 2020, an employee was looking for supplies in the crawlspace above the cooler when she fell through a ceiling tile, clipped a shelf containing dishes, and landed on the floor in the prep area approximately 12 feet below. She was admitted to the hospital for a fractured shoulder, two fractured ribs, and a collapsed lung.
