Incident Narrative
An employee was operating a standup clamp truck, transporting a large box (washing machine) across a facility. As the truck passed a structural support beam, the corner of a clamp at the back struck the beam. The resulting jolt caused the employee to fall out of the truck, which continued to roll forward and struck the employee's foot. The employee suffered a partially amputated big toe.
