Incident Narrative
A crew was using a hydraulic tunneling jack to push a carrier pipe into a tunnel. An employee was adjusting the rigging on the tunnel jack machine when the push plate became dislodged from the end of the pipe and fell. The plate struck the employee and came to rest on the backside of the tunnel pit. The employee sustained a compound fracture of the left humerus, a fractured right ulna, and lacerations to the head and knees.
