Incident Narrative
An employee was cleaning an encapsulation machine when her left index finger got caught between two segments of the machine and was amputated. An employee was working on a granulator machine to align the hopper when his right middle finger got caught between the hopper and the base of the machine resulting in a fingertip amputation.
