Incident Narrative

An employee was in a trench that was approximately five feet deep, installing grout and bricks around a pipe that lead into a concrete drainage structure. The employee was between the concrete structure and a steel cave-in protection shield. An excavator was operating close to the trench and caused the steel shield to move, which pinned the employee's hip against the concrete structure. The employee sustained a fractured right hip.
