Incident Narrative

An employee was removing electrical conduits and a junction box from the exterior of a building. While on a 12-foot A-frame ladder, the employee removed the cover from the junction box and was loosening the anchor bolts within the empty junction box when the entire box and six conduit pipes broke free from the wall and struck the ladder. The ladder tipped over and the employee fell approximately 9 to 10 feet onto the asphalt ground, breaking his ankle. The conduit pipes then fell on him. The broken ankle required surgery.
