Incident Narrative
An employee was on a flatbed truck and was using a center-mounted conveyor belt. As the employee was loading a cube of foam insulation onto the conveyor belt, the cube rolled back onto the truck. The employee stepped to the side to avoid the cube and fell 52 inches off the truck to the ground. The employee sustained a broken right leg and a bruised right shoulder.
