Incident Narrative

An employee was putting a bend channel in a sheet of metal using a press brake. The brake actuated and the sheet metal rose up against the brake die, smashing the employee's fingers between the brake die and the sheet metal. The employee suffered a partial amputation of the left middle finger at the last joint.
