Incident Narrative

An employee was operating a machine that worked with extruded plastic. The grinder attached to the machine became clogged and the employee stopped the machine to unclog the grinder. As he was clearing the jam, the grinder motor was still winding down and the still rotating blades inside the equipment amputated multiple fingers.
