Incident Narrative

An employee fell through a 48-inch opening in the roof where two panels had been removed and struck a shelving unit and then landed on the floor approximately 30 feet below. The employee suffered fractured ribs, a punctured lung, and a bruised liver. The employee was not tied off at the time.
