Incident Narrative

Employees were inside the basket of the feed truck repairing the drive shaft on the main floor chain that is beneath the grain pile and perpendicular to the feeder/delivery chain. As the employees were removing feed from the main floor basket to access the drive shaft, an employee was swept by the feeder chain and pulled up and entangled into the beater bar. The employee was hospitalized with a lacerated chest, punctured lung and broken left tibia.
