Incident Narrative
An employee was attempting to close the extended lip of a malfunctioning dock plate. He used a T-bar to prop the dock plate up while trying to disengage the dock lock with another T-bar. The T-bar propping up the dock plate fell, causing the dock plate to fall down on his left arm and break it.
