Incident Narrative

An employee was changing the plunger on a press and adding bolts to the side of the plunger. When the employee lowered the lazy Susan, the plunger fell and one of the bolts punctured the employee's right hand, causing an open fracture. The employee was hospitalized, requiring surgery.
