Incident Narrative

An employee was spray-painting the ceiling and upper walls of a dock area. He was working from a heavy-duty plastic utility storage bin/box placed on the forks of a forklift. The box shifted, tossing him to the concrete floor about 9 feet below. He suffered broken ribs.
