Incident Narrative

An employee was attempting to bend back a loose pulley onto the mast of a powered industrial truck when the employee made contact with the lever. The mast then tilted back, causing the employee to become caught between the truck's mast and overhead guard. The employee sustained abdominal injuries and intestinal bleeding.
