Incident Narrative

On 7/14/2020, an employee on a loading dock was scanning a barcode that was attached to the rear door of a trailer. The employee lost her balance and fell 45 inches from the dock to the cement ground. The employee sustained a broken left arm, requiring hospitalization and surgery.
