Incident Narrative

Two employees were in the bed of a customer's pickup truck, moving a grill to the back of the bed. The customer removed his ratchet straps from the truck's cab and threw them on the ground. When one of the two employees stepped down from the truck bed, his left foot landed on top of the ratchet. The employee went to the ground and the foot was broken.
