Incident Narrative

An employee was operating a narrow-aisle standup powered industrial truck (PIT) in the manufacturing warehouse. When he attempted to slow down, the lift started to slide due to the damp floor from the rain. His left foot became pinched between the PIT and a steel bollard. He sustained an open displaced fracture with a laceration of the left big toe.
