Incident Narrative

A temporary employee was cleaning excess oil from around the air operator machine (punches a hole in material). When the employee inadvertently contacted a button located below the table, the air cylinder moved down and caught the employee's left middle fingertip between the vertical guides, resulting in fracture and partial amputation. The machine was not guarded.
