Incident Narrative

An employee was starting the hammer mill line from the control station. Once the start button is activated a short delay occurs before the machine starts moving. The employee then ran down two flights of stairs, lifted the rubber cover on the chute for the elevator bucket, and was caught by the equipment as it started to move. The employee sustained fractures to the ulna and radius in his right forearm and the right index finger was amputated to the second knuckle.
