Incident Narrative

An employee was operating a bottle blow molding machine when he dropped his cell phone through the front opening of the machine. He activated the machine to move forward and attempted to retrieve his cell phone. While bending over, he touched the stop button, which caused the machine to reverse into the home position. The employee then became pinned when the panel moved into the home position, injuring his left arm and upper torso on the left side.
