An employee was elevated in the bucket of a back hoe and was using a bottle jack to straighten the body of a dump truck under repair. The bottle jack became dislodged and struck the employee in the face. The employee was propelled backwards and hit the back of their head against the bucket of the back hoe, sustaining a head injury.
