Incident Narrative

An employee was walking from the maintenance shop to the warehouse area. He was struck in the back by a sit-down forklift that was carrying two stacks of oriented strand board (OSB) that had been recently heat-treated. Each stack weighed approximately 3,500 pounds and had a temperature of approximately 250 degrees Fahrenheit. The employee sustained burn injuries to his lower back and buttocks, a puncture wound to his right forearm, and contusions to his ribs.
