Incident Narrative

An employee was closing a cardboard baler with his left hand. He pulled down the door and continued to hang onto the handle of the cardboard baler with his left hand as it closed. He reached with his right hand to start the baler. As he did this, his left pinky and ring fingers were caught in the baler, which started to pull his fingers in. His fingers were then caught on the safety mechanism.
