Incident Narrative
An employee drove a forklift into the aerosol room with a pallet of product on the forks. She drove down the back driveway to the aisle of racking she needed to turn into. The forklift drove through salt brine that was on the floor and slid into a concrete bollard at the end of that aisle. The employee's left foot got caught between the bollard and the forklift. The employee was hospitalized with four broken toes and a laceration to the left foot.
