Incident Narrative
An employee was cleaning his work area when the strap holding a container rotator in a vertical position failed. The container rotator, and the container roll-in/out platform it was attached to, swung and struck the employee on the right upper torso and hip area. He suffered broken and dislocated ribs, a pelvic fracture, and fractures to lower-back vertebrae.
