Incident Narrative

A team of three employees were removing wood panels from the wall. Two workers were at the top of the panel and one was at the bottom. The panel weighed approximately 90 lbs. It slipped out of the hands of the workers at the top and landed on the hand of the employee at the bottom severing the employee's left ring finger.
