An employee was jackhammering a concrete deck on the top level of a parking garage when the deck collapsed and he fell to the level below. He impacted concrete rubble on the floor and a piece of concrete slab that sheared from the deck, suffering a fractured femur, broken tibia, and cracked ribs.
