Incident Narrative

An employee was working on a roll-up door in the maintenance shop when he noticed that the chain was loose on top, so he placed a ladder next to the door to climb up and fix it. The ladder was placed on the bottom lip of the garage door. The employee asked another employee to push the button to have the door roll up so he could monitor the movement of the door/chain. As the door rolled up, it pushed on the ladder, causing the employee to fall. He struck the ladder, a nearby object, and the concrete floor, injuring his back, side, leg, and arm.
