Incident Narrative

An employee was preparing to fill a truck up with water at a water station. The truck had a 3-inch cam lock and when the employee attempted to make a connection, the hose popped off, striking the employee in the face with approximately 700 PSI. The employee sustained a concussion, broken nose, and facial lacerations, requiring hospitalization.
